Application letter (handwritten and signed on stamp duty Rp 6,000), addressed to Direktur Utama RSJ Dr Radjiman W Lawang;
Copy of legalized diploma, with the minimum accredited C (1 sheet);
Passport photograph size 4×6 (2 sheets);
Curriculum Vitae (CV);
Copy of ID Card (1 sheet);
Minimum age of 23 years and maximum 35 years old by 1 February 2016 (Tenaga Perawat, Asisten Apoteker and Apoteker).
Maximum age of 45 years old by 1 February 2016 (Tenaga Medis).
Have Certificate Register (STR).
Have one of the following certificate : ACLS / ATLS / GELS / BLS (Perawat and Dokter).
Medical certificate from government doctor.
SKCK from local police office.
Affidavit does not require a civil servant and signed on stamp duty Rp 6,000 (according to the attached example);
Working reference (if any).
